After 14 years co-chairing the Victoria and the South Island Divisions’ Care Transitions Steering Committee, Dr Lisa Veres is stepping down from the role. She has been spearheading the work from the outset, when community family doctors were struggling to stay informed about what happened with their patients in hospital. She also served on the VDFP Board from 2014–2017.

Joined since the early days by Dr Laura Phillips as co-chair, the launch of e-notifications marked the first breakthrough. Funded by Shared Care and developed through collaboration between the Victoria and the South Island Divisions’ family physicians and Island Health, the new system closed a foundational communication gap and was eventually adopted provincially.

Since then, the Care Transitions team has continued to improve communication and coordination across the health system, from physician connectors supporting team-based care for vulnerable populations, to the Familiar Faces and Opioid Using Patients protocols supporting frequent ED visitors, and the Hot Hip protocol for suspected fractures in long-term care residents.
