The vision for the Victoria PCN is a well-supported local provider network that will, over time, enable all residents to access timely, comprehensive, culturally safe and coordinated team-based care.
The Victoria PCN is in year three of its four-year implementation plan. Participating FPs are teamed up with MHSU health consultants, primary care clinical pharmacists, and social workers. In addition, we support clinics interested in collocating an RN into their practice.
Our PCN is a collaboration between VDFP, Island Health, Indigenous Partners and Patient Partners.
