Program Manager – Long-term Care Initiative
Job Summary
This is a temporary position covering a leave. The employee on leave is expected to returning between July and December 2025. Reporting to the Executive Director, the Program Manager maintains effective and efficient processes and procedures to ensure the on-going operation of the LTCI including: budget preparation and management; HR infrastructure needs; operations planning and monitoring; quality improvement and risk management plans. The Program Manager assumes day-to-day executive responsibilities for the program, including supervising other staff and contractors. The Program Manager builds collaborative relationships with team members, family physicians, other division staff, and community and health authority partners.
Duties & Responsibilities
- Coordinate the duties and responsibilities of the program team to fulfill the LTCI deliverables, and meet regularly with the team to provide support, monitor progress and participate in program activities, where necessary and appropriate
- Plan work, allocate resources, define tasks, assign responsibility, monitor quality and performance
- Work with LTCI Steering Committee Co-chairs and physician leads to operationalize strategies to meet Vision, Mission, Key Focus Areas
- Support the steering committee as well as any and all working groups that may develop
- Coordinate aspects of program evaluation as required
- Provide oversight and operationalize LTCI communication plans
- Provide operational direction and troubleshooting regarding TORCH and CORE implementation to coordinators
- Quality Improvement: Assist with ongoing quality improvement (QI) cycles between physicians and long-term care facility partners; develop project supports according outcomes of QI cycles
- Operationalize direction for all aspects of Practice Support: physician data collection and payments; learning series; special incentives; parking passes; billing support
- Coordinate all aspects of After-hours call group (liaise with dispatch service, collect and analyze data, scheduling, physician credentialing, data reports, etc.)
- Supervise data management and analysis and analyze program participation figures
- Partner relationships: Facilitate orientation for new LTCI physicians, including LTCI opt-in processes and physician-facility agreements; assist with initial contact and orientation of long-term care facility directors of care and managers via email, telephone, and in-person meetings
- Provide operational support for System Coordination and Sustainability work
- Prepare a monthly progress report on all program activities, and assist with developing division reporting forms and processes as needed
- Sign-off on LTCI expenses and review monthly financial statements
- Ensure that the security and confidentiality of all data is maintained consistent with provincial regulations and division policy
- Other duties as required
Qualifications
- Bachelors degree in social sciences, healthcare, or business administration, or an equivalent combination of education and experience
- Minimum five years recent related operations experience, preferably in the non-profit or healthcare sector
- Knowledge of the BC healthcare system and experience working in a long-term care setting or experience working with physicians and/or allied health care providers
- Collaborative team leadership style with exceptional interpersonal communication, relationship building and problem-solving skills
- Excellent organizational skills and ability to prioritize and manage multiple tasks to meet quick deadlines with quality output
- Excellent verbal and written skills
- Able to work independently and as a member of the team
- Excellent computer skills, particularly in Outlook, Excel, Word and PowerPoint
Conditions of Employment
- Full time (37.5 hours per week) – hours determined collaboratively with supervisor
- Extended benefits offered after 3 months’ probation
- May work from a remote, secure worksite and/or at the Division’s downtown Victoria location
- Attendance at evening meetings is required
- A valid driver’s license and access to a vehicle is preferred
