Large interdisciplinary team including full time social worker, 4 full time equivalents of nursing, an occupational therapist and a dietitian all working collaboratively with the GPs and NPs to support patient care. Service contract for longitudinal care, no overhead. 8 exam rooms, team room with 7 workstations – please contact for a tour.
