Practitioner Connectors can make your referrals to Island Health Community Health Services much easier. Experienced nurses offer support regarding home support, nursing, physiotherapy, and occupational therapy. They can discuss functional or clinical needs and desired outcome, and process referrals for Assisted Living, Long-Term Care, Respite, and Adult Day Program. Call them directly at 250-519-5282.
