Primary Care in Victoria
Family medicine-trained physicians in Victoria provide comprehensive, patient-centered health care, focusing on patient wellness and quality of life, as well as physical or mental health concerns.
They play an important role in Victoria’s health care system, acting as the central hub for managing health or illness for patients and families, and providing referrals to specialists when needed.
Our family medicine-trained physician members work across many different areas of care, depending on the needs of the local community. Areas of care include:
Long-term Care
Mental Health & Substance Use (MHSU)
Senior & Frail Elder Care
Family physicians also work in a variety of practice settings including:
Patient Medical Homes (PMHs)
Walk-in Clinics
Urgent & Primary Care Centres (UPCCs)
Hospitalist Environments
Emergency Departments
Community Health Centres (CHCs)
Long-Term Care Homes
In a growing number of practices across the Victoria Primary Care Network, team-based care is being provided. These practice teams include other health care professionals such as registered nurses for patient support and case management, MHSU consultants, social workers, clinical pharmacists and Indigenous wellness providers.
Family medicine-trained doctors provide leadership in practices, communities, and in the health care system. Family physicians work with partners, including health authorities and other local health care providers, on initiatives that will help make family practice sustainable and thus improve access to health care services and foster better health for patients and communities. This is the core of our “Happy doctors, Healthy communities” mission.
