The Victoria Division’s Care Transitions Committee and Island Health collaborated to improve information flow from CHS to FPs through automated, regular reports distribution. This process is similar to the one for discharge summaries and other documentation.

To better meet FP information needs, CHS clinicians will now use the Provider Communication Note in PowerChart to share key updates, including:

• when a patient starts receiving CHS services
• the services the patient will receive and from which disciplines
• contact information for the most responsible care team
• changes in the patient’s health status
• updates to the patient’s care plan
• when the patient is discharged from CHS

These timely and consistent updates will better support coordinated patient care. Provider Communication Notes will be sent according to your existing report distribution preferences, ensuring you receive information as expected.
