Re-enrolling patients in Community Virtual Care
Did you know? You can refer patients to Island Health’s Community Virtual Care (formerly Home Health Monitoring) program more than once. If they received CVC for chronic disease self-monitoring (e.g., heart failure, kidney disease) and still need support, they can be re-enrolled.
Update – Perinatal services, after hours coverage, engagement opportunities
