Feedback on information received from Community Health Services

Island Health wants your feedback on recent updates to the Community Health Services (CHS) Provider Communication Note. Have these changes made it easier to know when community services are initiated, who to contact, and to receive care plan or discharge updates? 

The Note, used by CHS clinicians to share non-urgent patient information with family physicians, was improved earlier this year to include notification when a patient starts CHS services, what services and disciplines are involved, contact details for the most responsible care team, changes in health status, updates to the care plan, and discharge information.

Notes are sent according to the delivery method selected in PowerChart (Excelleris, Message Center, or Mail). If you don’t receive these notes as expected, preference settings may need to be reviewed.
