Long-term Care Initiative

Victoria South Island Long-term Care Initiative (LTCI) Program

The provincial LTCI is designed to optimize medical care and address challenges in long-term care (LTC), by enabling physicians to develop local, scalable, and sustainable solutions. Locally, the Victoria-South Island LTCI works to improve the care of all residents in LTC homes, through implementing best practice expectations for medical care, collaborating with physicians and care home teams, and facilitating system-level outcomes intended to change the culture and structure of LTC.

LTCI Best Practice Expectations
  • The LTCI’s best practice expectations (BPEs) are designed to improve medical care for residents, as well as to create positive change in the culture and systems of long-term care. To support physicians and care home teams in meeting the BPEs, we’ve created a toolkit with practical information, templates, and other useful resources for all six BPEs
  • 24/7 availability and on-site attendance, when required,
  • Proactive visits to residents,
  • Meaningful medication reviews,
  • Completed documentation,
  • Attendance at case conferences, and
  • Participation in a regular quality improvement process.

  2. Opt-in to the program by signing a system-wide LTCI Letter of Agreement (LOA) that covers all residents for whom you are MRP.
  3. Access LTCI Benefits and Incentives, including regular LTCI payments.
Local LTCI status update

The Vic-SI LTCI is active at all 37 local long-term care sites, with 68 LTCI physicians acting as MRP for 93% of all 3,441 local residents. These 68 physicians represent approximately 69% of all MRPs practicing in LTC. 100% of residents are covered by LTCI after-hours call groups in Victoria, Sooke, and the Saanich Peninsula.
