The Shared Care Committee of Doctors of BC has provided initial project funding to engage primary care physicians interested in working with specialists in discussions around the development of a collaborative model of care for patients with cardiometabolic risk factors and disease. The project aims to obtain feedback needed to develop this integrated model of care aimed at improving clinical outcomes, accessing care in a timely way, enhancing patient education and providing administrative and clinical support for the physicians involved. Clinical outcomes and patient and physician satisfaction will be evaluated.
