What is a PMH?
The Patient Medical Homes (PMH) model describes how a medical practice can be optimally designed to meet the needs of the patients, the providers, and the community. Implementing the optimal design in a practice setting provides access to quality primary care. How the attributes are achieved is likely to vary in interpretation throughout the province reflecting the diversity of communities and practices.
The good news is that the vast majority of practices in BC are PMHs of some form. We believe that by supporting practices and providers to explore and develop the PMH attributes we will improve access to quality primary care, and by extension, enhance patient and provider experience of the system.

The MOA Network supports the evolving needs of MOAs working in Victoria PCN member clinics through a combination of clinical and professional development supports, resources, and ongoing engagement and education opportunities. Visit our MOA Network page for more information.
In collaboration with the Island Medical Program and the South Island Division, the Victoria Division actively engages with medical students to support and encourage choices pro family practice. Activities range from student socials to specific supports to forums with practicing family physicians.
The Victoria Primary Care Network is envisioned as a well-supported local provider network that will, over time, enable all residents to access timely, comprehensive, culturally safe and coordinated team-based care. Are you interested in exploring new tools or team-based care options that could improve your practice experience and patient care? If so, you can access customized coaching, and mentoring services directly in your office through the Practice Support Program (PSP).
The Urban Locum Program (ULP) Greater Victoria Pilot aims to offer up to 20 days of coverage over the duration of the pilot, for eligible family doctors and a career pathway for new physicians into longitudinal care.
